John Lewis Rees Dymuna teulu y diweddar Mr John Lewis Rees, Nantyrefel, Boncath, ddiolch yn ddiffuant am bob arwydd o gydymdeimlad a charedigrwydd a estynnwyd iddynt yn eu profedigaeth. Diolch arbennig i'w Ymgynghorwr Parkinson yn Ysbyty Glangwili (Dr S. K. Gupta), Meddygon a Nyrsys Cymunedol Meddygfa Emlyn, a'r holl Ofalwyr, am eu gofal caredig yn ystod ei waeledd. Diolch diffuant am yr holl gyfraniadau er côf am John tuag at Gronfa Parkinson Ysbyty Glangwili, Cronfa Nyrsis Gymunedol Meddygfa Emlyn, a Sefydliad Paul Sartori.
